Repeat Prescriptions

Are you ordering your medication at different times throughout the month? Now you can request to have your medications aligned to save time for you, your doctor and your Pharmacist.

Click here to download the guidance and form for completion. Please return your completed form to reception marked FAO Practice Pharmacist. Thank you

prescriptionsAll requests for repeat prescriptions must be in writing, using the prescription counterfoil, surgery order form or other written means. This can be left in the Practice Prescription boxes in the main Surgery entrance hall.

PLEASE ENSURE YOU NOMINATE A LOCAL PHARMACY TO COLLECT YOUR PRESCRIPTION. PLEASE SPEAK TO RECEPTION STAFF IF YOU HAVE NOT ALREADY SET THIS UP.

Telephone requests for repeat prescriptions will not be accepted.

Please ask a member of staff to set this up for you - you will need an email address for each person.

PATIENT ACCESS - THIS IS A FACILITY WHERE PATIENTS MAY ORDER THIER REPEAT MEDICATION ONLINE - PLEASE ASK AT RECEPTION.  YOU MUST PROVIDE AN EMAIL ADDRESS TO SET THIS UP.  IT IS VERY EASY TO USE.

Requesting prescriptions on line is for repeat medication only. Please do not request acute medication or send messages through this online facility.

Forty eight hours notice is required for all repeat medications.

Please allow sufficient time for your nominated pharmacy to collect & prepare your script ready for collection. Please understand our Pharmacy colleagues are under a huge amount of pressure currently. If your medication is non urgent please allow an extra day before calling to the Pharmacy.

Only you, a family member or carer can order your repeat prescription for you.

Requests from pharmacies on your behalf will not be issued.

Collecting your prescription:

Currently patients should nominate a local pharmacy to collect their prescriptions due to reduced service at The Surgery & Social distancing etc

If ordering online or leaving your printed order script to The Surgery, you must add a nominate chemist at the bottom.  Please add a contact telephone number either in the 'text box' online or on the bottom of the repeat order slip.  Please note by adding this to the script request you are consenting to us sharing this information with your nominated pharmacist.  This is incase they would have an issue with your medications and should need to contact you prior to you calling at the Pharmacy.

Thank you for your understanding.
